Submissions
- Tissues should be submitted in 10% neutral buffered formalin.
- Use this link to request biopsy submission materials online. You may also download the pdf version here or under “Forms” and fax your supply request or include with your biopsy submission
- To submit biopsies, use this link to fill out our online submission form and submit it directly from our website. If you use the electronic submission form, make certain biopsies are clearly labeled with the owner, patient and clinic names.” You may also download and print our submission form here to fill out and include with your biopsy.

Step by Step Process

- Step 1. Place specimen in tightly sealed formalin-filled container
- Step 2. Place formalin container inside sealed plastic bag
- Step 3. Place plastic bag in crush proof cardboard box with absorbent towel
- Step 4. Please combine multiple biopsies in one mailing box
- Step 5. Tape box closed and place in stamped plastic mail pouch
- Step 6. Apply return address and seal pouch!
- Step 7. Mail via US Postal Service–confirmation delivery may be added at your post office
